In order to attend Alcoa City Schools, there are specific documents that are required to be on file. According to Alcoa City School Board Policy 6.203, all items listed below must be in cumulative files to attend school on the first day.
  • Proof of birth _____

  • Proof of Residence _____

  • Social Security Card _____

  • Immunization Record _____

  • Evidence of Medical Exam _____

  • Parent/Guardian ID ______

  • A signed Home Language Survey _____ (please come to the office to complete)

  • A signed Migrant Survey _____ (please come to the office to complete)
